Application Forms
The main forms required for an application to Cedar International School are available for download below. In addition, please see the checklist for additional items required below.
Please provide the following items, in addition to the completed forms contained in the package above.
- Non-refundable $300.00 Application Fee
- Copy of most recent Grade Reports (2 years)
- Copies of academic or relevant health assessments and/or SEN documentation.
- Copy of most recent Immunisation Record
- Copy of Birth Certificate & Passport page/Belonger Card
- Copy of permission letter from the Department of Education
- Two passport size photographs
After an acceptance letter has been received from the school, you will be expected to complete and return the enrolment contract (below) along with payment of pre-enrolment fees to confirm your place.
